Welcome to Glenwood High School!  We're glad you're here. New students and their families are welcome at any of our schools to tour our facilities and meet our staff. When enrolling, please have the following forms ready to present to Glenwood High School:

  • Student record release form

  • Home language survey

  • Copy of birth certificate

  • Copy of current physical and proof of immunizations (see medical information below)

  • Residency forms

  • Student in Good Standing transfer form

  • Test scores (Achievement, IAR, as appropriate)

  • Transcripts from the previous school

  • Social Security Number

  • Registration Fee - Grades 5-12 $155

Medical Information and Forms

All new students must have a current physical and proof of immunizations. They are also required to show proof of an eye exam performed by an Illinois licensed optometrist.

Required for K-12 Students New to the District

  • Physical Exam: All new students entering any grade must have a current physical and provide proof of up-to-date immunizations. Students without a current exam on file will not be allowed to attend the 1st day of school unless there is an appointment card from the doctor’s office on file with the school nurse.

  • Dental Exam: Illinois law requires a current dental exam for 9th graders. The dental exam is to be turned in by May 15th of the current school year.  It can be dated up to 18 months of the May 15th deadline. If you are without dental insurance or cannot afford the exam you will be required to fill out the Dental Exam Waiver for their child.  The waiver form will fulfill the state requirement.

  • Eye Exam: all students entering an Illinois school for the first time will need an eye exam by an optometrist or physician who provides complete eye exams. The eye exam is due by October 15th.

  • DPT: (Diptheria, Pertussis, Tetanus) four or more doses required with the last being a booster that was received on or after the 4th birthday.

  • Hepatitis B: three doses required. The first two doses are no less than 28 days apart and the interval between the 2nd and the 3rd doses must be at least two months apart.

  • MMR: (Measles, Mumps, Rubella) 2 doses required.  The 1st dose on or after the 1st birthday and the 2nd dose no less than 28 days after the first.

  • MCV: (Meningococcal) All students over the age of 11 years old are required to be vaccinated with a single dose of a quadrivalent Meningococcal vaccine.

  • OPV/IPV: (Polio) three or more doses are required with the last dose received on or after the 4th birthday. Any combination vaccine with IPV requires four doses.

  • Varivax: (Chickenpox) two doses required.  The 1st dose on or after the 1st birthday and the 2nd dose no less than 28 days after the first. OR PROOF OF DISEASE

  • Tdap: (Tetanus, Diphtheria, Acellular Pertussis) 6th grade requires one dose of vaccine.

Required for Ninth Grade - 

  • Complete list of all immunizations

  • Physical Exams: Current State of Illinois school physical examination (Sports physical forms cannot be accepted); Current physical is an exam dated less than 1 year from the 1st day of school

  • Vision Exam: performed by a licensed Optometrist or Ophthalmologist (only if a student moved from another state)
